  • Possible HAI (i.e. patients admitted 3 days ago where WBC is not in the range 4,000-12,000)
  • Drug/bug mismatches (bug identified is resistant to patients current antimicrobial therapy)
  • Positive MRSA screen that are NOT on Vancomycin, Linezolid (or another antibiotic)
  • Deep wound cultures positive AND patient is on Cefazolin
  • Blood culture contains Pseudomonas AND drug name equals Zosyn
  • Gentamicin peak > 10ug/mL AND patient is in the ICU (or other service area)
